The expression tall drink of water is usually used when talking about handsome or attractive men. In my mind, the expression is associated with cowboy-like men or rugged-loooking men.

If you see someone attractive, you can say “Look at that tall drink of water!”, or “Hey, handsome. Aren’t you a tall drink of water!”
In the example above, actor Kevin James is saying that actress Amber Valletta is hot or very attractive. Who do you think is a tall drink of water?

In the U.S, usually said to or describing a tall good-looking man, but can refer to a tall woman.  A common expression among southerners, or people with family from the south. Based on the concept that in the heat, a tall drink of water is very refreshing and desirable.  Not considered slang, but a compliment.  It is not "dumb", timchangbest, as it is complimentary, can be said by an older person to a younger person of the opposite sex (usually a close relative or neighbor) and is a polite way of telling someone they look good.
